    I believe that's what's known as a bidet - they're still quite common in Europe, especially in hotels, which is where I imagine this shot was taken. In any case you can tell it's not a second toilet since the front doesn't come out quite as far as the one she's not sitting on.
    (If those are in fact 2 toilets next to each other, and of different sizes, that would indeed be the most eccentric if not incompetent bathroom design I've ever seen.)

    Bidets are great if showers are unavailable.

    It’s good to shower every day, though. Washing the taint isn’t a substitute for a shower.
