I have no idea how many people are typically involved in putting the photoshoot together, but I would think there would be a separation of duties. Someone creates the concept, another person does wardrobe, another handles makeup and the photographer takes the pics. My guess is the person who conceived of the idea for the shoot is responsible. If I were any of the others involved, I'd just be worrying about my job and nothing else.

There is a huge group of people involved in publishing a major magazine - the photo shoot is only the start.

After the photographer takes all the photos, usually 100 or more with different expressions, positions, and lighting. The editorial staff will chose which photograph to use. The selected photo is then sent to the computer graphics department which will remove the imperfections in the background as well as any blemishes, beauty marks, zits, etc. The editorial staff will be involved here as well deciding what and how much to edit.

Then a designer will choose the fonts for the headlines and all the other text and determine a layout for the cover - after that the computer graphics department will create the cover all the while the editorial staff will request changes which the graphics department will make and then resubmit to the editorial staff for final approval (this process can take many iterations).

At any time in this process, any editor, publisher, graphics dept head, column writer/reporter or proofreader or anyone else that saw the cover could have pointed out the error and a computer artist could have photoshopped out the signature in a few hours or less - but no one noticed, or cared that it was wrong.
