    I think he's looking at the crease in her tummy between her belly button and the top of her bikini.

    A true C-section scar would be right around the level of the top of her bikini or just below it. The surgeons try to make it as low as they safely can for the sake of the woman's aesthetic appeal so that she can actually wear bathing suit bottoms without showing the scar. Of course, the reason for the C-section has a lot to do with its location - elective versus emergent, and size of the baby plus the position of it in the uterus is taken into account.
