    Here is a simple way for you to try it yourself rapid.

    1. Open the image in your photo editing software (photoshop or whatever)
    2. Create a new layer an top of the image.
    3. Use the paint pot to fill the entire image.
    4. Lower the opacity in the layers blending options so you can see what you are doing. But not so far that you cannot see the fill on the top layer.
    5. Use the eraser tool to erase circles on the image, avoiding any clothed area. The closer to the more revealing areas, the better the illusion.
    6. Raise the opacity of the fill layer to see if you are happy with your work.
    7. Flatten and save. If you aren't happy continue erasing.

    There's another way too, kind of the opposite way jamms described.

    Get circle brushes and paint on all the skin without brushes overlapping, select all the circles, invert the selection, fill it with your desired colour, delete the circles.

    I do it as jamms described, but it can be done other ways to suit your preferences.

    Maybe a couple of pictured instructionals would be good. I'd put them in the OP.
